Article 86

The following documents must be annexed to the application:

  1. 1.Proof of identity of the applicant and the designer
  2. 2.A sample of the shape or graphic image, or a drawn representation of the various aspects of the design for which protection is requested
  3. 3.Submission of a maquette (model) or actual specimen of the design, where the Registration Authority so requests
  4. 4.Description of the design and designation of the goods on which the industrial design is to be applied
  5. 5.A written request that the designer's name not be mentioned, where he does not wish his name to appear in the application
  6. 6.The document relating to the priority right, which must be filed simultaneously with the application or at most within one month from that date (where priority is claimed)
  7. 7.Receipt for payment of the statutory fees
